  Your consent is required to allow us to pass your details, in confidence, to other organisations regarding matters relating exclusively to this application.

• I agree that, in order to initiate my application, my Service details will be checked with the RAF Records Office. I also agree that in order to process my application certain details that I have provided may be disclosed to various organisations including the Department of Work and Pensions, hospitals and GPs. Additionally, I understand my details may also need to be disclosed to other charities (including religious charities) and similar organisations for the purpose of possible additional funding for my application. Where any person upon whom my eligibility is based is currently resident abroad, I understand it may be necessary to send my details to that country for verification purposes. I understand that this form is used to seek advice on how to proceed with a formal application but does not guarantee I will be entitled to support from the RAF Benevolent Fund.
