2026 Serving Breaks application form

This form is for serving RAF people applying for a break with their families from 11 September to 11 December ONLY. The form will take a few minutes to complete. Other types of assistance can be accessed by speaking with your unit welfare team.

 

It is important to provide us with proof of current RAF service to enable us to process your application. Please attach a photo or scan of your MOD90.
