We need this information for administrative purposes and will only use your contact details to get in touch with you about the event you have signed up for, unless you choose to receive email updates showing how your support is helping by ticking the box at the end of this form.

By completing this form, I confirm that I agree to the following:

  1. I acknowledge that any funds I raise should be paid over to the RAF Benevolent Fund within six weeks of the event end date.
  2. I acknowledge that I am taking part in this event at my own risk and that the RAF Benevolent Fund will not be held responsible for any injury or illness resulting from my participation in the event. I will undertake suitable training and consult my doctor if I have any health concerns.
