Before starting this form, please ensure you carefully read the below guidance. Please note, it may take you between 10-20 minutes to complete this form.

Guidance notes

This application process can only be used once by an applicant. The form is for one-off financial assistance, we will consider your needs and may support you with up to £750 or less. Please note this application form cannot be used to seek assistance with non-priority debts, legal costs, property adaptations, care costs or assistance with your care or mobility equipment. Other forms of assistance can be accessed by calling 0300 102 1919.

To proceed with this form, please ensure you have the following information to hand of the person who requires support (this will be your details if you are applying for yourself):

  • Bank details
  • Weekly household finances

You will also need the following documents of the person who requires support in electronic format to upload and submit to us:

  • Proof of ID (e.g. driving licence, passport, bus pass)
  • One month's bank statements dated within the last months
  • Universal credit statement/benefits (if you are in receipt of this)
  • Proof of RAF Service
  • Quotation/Evidence of Priority Debt/Invoice (if seeking support with living costs this is not required)
